I understand that hair and makeup services may involve certain risks, including but not limited to , chemical treatments, use of styling tools and application of various hair and makeup products. I hereby agree to the following: 1. Assumption of Risks: I acknowledge and accept the inherent risks associated to you beauty care services, including but not limited to, damage to hair, scalp/skin irritation, allergic reactions, and dissatisfaction with the final results. I voluntarily assume all risks. 2. Accurate Information: I confirm that I have provided accurate and complete information may increase the risk of adverse reactions. 3. Results: I understand that the results of beauty services can vary based on individual hair/skin types, current condition, and other factors. TheGodDez1 will make every effort to achieve the desired outcome, but results are not guaranteed. 4. Liability Waiver: I hereby release and hold harmless TheGodDez1 Beauty & More and it's assistants from any or all claims, damages, or liability arising from or related to the beauty services provided, including but not limited to bodily injury, property damage, or dissatisfaction with results. 5. Follow-Up Care: I agree to follow the recommended aftercare instructions provided by TheGodDez1. I understand that failure to adhere to these instructions may result in damage to my hair, skin or scalp, for which TheGodDez1Beauty & More is not responsible 6. Emergency Medical Treatment: In the event of and allergic reaction or medical emergency, I authorize TheGodDez1 to seek immediate medical treatment on my behalf if necessary. 7. Legal Age: I confirm that I am of legal age to consent to these services, or I have obtained consent from a legal guardian if required. 8. Dispute Resolution: Understand NO REFUNDS will be granted, services do not have to FIXED the same day if you do not like your services. TheGodDez1 can refuse servicing any person. In the case of threatening, fighting, arguing or aggressive behavior Dekalb County Police will be contacted.
